Complainant's Statement of Police Personnel Misconduct Form The complaint statement / form should be used to report allegations of police employee misconduct. Complaints of being arrested or receiving a ticket or citation are not within the scope of a complaint, unless it involves misconduct. Every citizen has the right to dispute an arrest or ticket in a court of law. Complaints are reviewed and investigated as to whether a police employee violated any rules, regulations, policies, or procedures. Please provide as much information as possible, including specific dates, times and the names of any police personnel, their badge number or car number, incident report numbers, ticket numbers and description of the officer and what color uniform he or she was wearing. Please be specific as to your complaint. It is important that you provide us with any names of witnesses and how they may be contacted. Depending on the information you provide, you may be contacted for additional information or be interviewed by the supervisor designated to investigate the complaint. The complaint form must be completed and signed by you. You are entitled to copies of your form and the department will make copies for you at no expense, if you deliver your form in person. You may make your own copies before you submit your form as well. You may mail the form or bring the form to the department, directing it to the attention of Chief Michael Galloway. Our address is: Macomb Police Department 120 S. McArthur St. Macomb, IL 61455 Upon receiving your complaint statement / form the investigation will be assigned to a supervisor in the department. The supervisor will be contacting you to advise you that your complaint has been received. The supervisor will be providing you with their name and how to make contact with them. The supervisor delegated the responsibility of conducting an investigation will contact you periodically to update you on the status of the investigation. The officer or employee(s) concerned will be informed of the nature of the complaint unless extenuating circumstances would dictate not informing the officer or employee. Upon completion of the investigation you will be notified of the disposition as to whether disciplinary action was taken or if none was taken, if it is determined that no violations of the rules, regulations, policies, and procedures were committed by the police officer or employee. Specific details of disciplinary action (i.e. hours or days of suspension) is not provided as this type of record is part of the employee's personnel file and not subject to disclosure to a complainant.
